What’s the name of this horror movie about a clown?

A long time ago I watched a horror movie, I’m pretty sure it was about a killer clown. From what I remember, there was a woman (or a man, not sure) living in a house in the middle of nowhere. There was a clown, before he appeared you could see cockroaches and hear music playing. He tortured people, I remember that someone got their tongue and fingers cut off. Also I think he had a room in the basement or whatever where he kept all his victims after he was done torturing them. That’s all I remember.
