Thriller where people are trying to get to a safe space…

I can remember very little about this film as I watched it YEARS ago, but remember a specific scene where a woman is lying in a sort of refugee camp in hospital beds (most people are passed out) after trying to escape “something?!”. She’s lying there as a man walks through the bed and starts impaling people… Really reminds me of the purge but can’t for the life of me think what it was called! Any thoughts? Thanks in advance!
