Old movie (80s – 90s) with a fat wedding at the end

All I can remember, is that there’s a wedding at the end, where the bride is fat. Maybe she was even carrying the guy down the steps, or maybe he was carrying her, but he was struggling.
I also remember that there was a car accident in the movie, where the woman was hit like 10 feet in the air. It’s a comedy, I think.
The main character, might have been a doctor.
