Depressed man with broken arm

Want name of movie about a depressed man with a broke arm who works in a store shoe department where he meets a woman who takes dying people to doctors appointments. He has two young sons who get blow jobs from two young girls. I think his estranged wife is black. Very dark movie. It was on regular tv late at night several years ago. Please.
